Become a Patient: What to Expect
When you become a patient at CVIM, you enter a healthcare home filled with passionate and compassionate caregivers. Here at CVIM, medical care, dental care, prescription drugs, social services, and health education are provided at no cost to uninsured, eligible patients. Patients receive comprehensive care, provided by skilled volunteers and strategic community partners, as we work to ensure a healthier community for all.
How Do I Become A Patient?
Step 1: It all starts with a phone call
If you have no access to health insurance and may need CVIM’s services, please call us!

Step 2: Complete a Phone Screening
A CVIM representative will schedule a phone screening with you to review your application and the process.
Step 3: Submit Your Documents Via E-Mail
After your phone screening, please submit the requested documents via e-mail to your designated CVIM representative.
