Complete Volunteer Application 
       [click to open; print; fill out paper form; submit to staff supervisor in your area of interest] 

Make Appointment 
       [contact staff in your Area of Interest to call or email to arrange for an interview]

Following your acceptance onto the Volunteer Team, you need to submit the following three documents:

        Criminal Check Approval 
        Child Abuse Clearance 
        Orientation Statement of Certification

Find forms under Volunteer Log In - User Name and Password available from your CVIM staff supervisor.
Quick Start Instructions - You can also request pamphlet available to assist you in completing all three forms.

Completed forms will be placed in your personal file and handled with confidentiality.
