Eligibility Requirement Minimize
CVIM, as a non-profit organization supported by private and public funds, has defined criteria for patient eligibilty.
 Those eligible for services must:
  • live or work in Chester County
  • meet gross income eligibility of 300% of the federal poverty guidelines or less*
  • not be eligible for Medical Assistance, Medicare, Veteran’s Benefits, or Children’s Health Insurance Program (CHIP)
  • not have private insurance.  However, if you have medical insurance and not dental insurance or vice versa, you may qualify for services provided all eligibility guidelines are met.

To Become a CVIM Patient
Follow the Steps Below:

These phone lines are staffed primarily by volunteers. 

Calls are usually returned within a day 
between 9:00 a.m. and 3:00 p.m. Monday through Friday.

 If you have to leave a message,
please make sure you give a number
where you can be reached during those hours
.
  

The volunteer eligibility screener will ask questions
about your household and your household finances.
 
You will be asked to bring household income
documentation** to your eligibility screening interview.

until all documents are received and reviewed.

Step 3 - Come to CVIM During Screening Hours Minimize

Screening Hours
You may come anytime during these open hours for eligibility screening:

English only:
Monday or Wednesday from 1:00 p.m. to 3:00 p.m.
Tuesday or Thursday from 9:00 a.m. to 11:00 a.m.

Spanish only:
Monday 12:30 p.m. to 2:00 p.m.
Tuesday 1:00 p.m. to 2:30 p.m.
Wednesday and Thursday from 9:00 a.m. to 10:30 a.m.

Horario Para Entrevistas:

Martes: 1:00 a 2:30 p.m.
Miércoles y Jueves: 9:00 - 10:30 a.m.

If you DO qualify for medical and/or dental services, an appointment will be made for you at a later date.

If you DO NOT qualify for services at CVIM, we will try to refer you
to other community agencies which may be able to assist you.
